Student Teacher Lesson Permission Form

 Hello Wonderful Families,

I have had the privilege of mentoring a student teacher AND Quest parent this year in our class classroom. On November 6th she will be teaching and recording a 1st grade science standard lesson to fulfill university credits. In order to facilitate this we need parent permission. The recording will be used for academic purposes only and WILL NOT be posted on any social media platforms.
